2011 W-9 Tax Form for Umpires All umpires must have a W-9 tax form on file with MCAA to get paid – No Exceptions! Click here to print and fill out a W-9 tax form. Please note: Umpires who worked/were paid last year do not have to complete another W-9 this year. However, all umpires need to keep their address information current with MCAA in order for their paychecks to get mailed properly.
